Please help very very urgent

I got my lloyds tsb Internet bank statements signed and stamped. But the stamp says that I CERTIFY THAT THIS IS A TRUE COPY SIGNATURE,DATE AND STAFF NUMBER AND NAME" But the stamp does not have bank name in it?Is that ok? But the statement contains bank logo and bank name. This is for visa extension.

please help Query on previous earnings

I am bit confused on the 12 month consecutive earnings. I am claiming earning from 2 employers, First Employer :1 AUGUST 2010 to 31 January 2011 Second Employer : 15 Feburary 2011 to 15 August 2011 There is a gap of 15 days,will this be ok. Will this be counted as 12 consecutive months? Please Help....

Appointment booking

After booking an appointment with PEO for Tier 1 extension
I see only main applicant details (Name,ref ID,trans ID) on the appointment letter.
Doent have and details of dependent is this ok?

Please let me know senior members.

I appled for hsmp in 8 Feb 2008 got my Hsmp approval in dated in March 2008,and applied for visa in September 2008.I am bit confused on the age points for extension.Please help...
