If it was before the application, maybe let them know. Since you've applied I would personally wait and if it raises any questions clarify it at that point. It could cause unnecessary confusion and delays if you send it now.

My understanding was the 6 month rule for payslips was a recommendation, but not absolutely essential, so maybe you can apply earlier.

Hopefully a more senior member can confirm or deny.

Can you provide the wording of the letter you received here.

The experienced members might be able to determine what the HO mean from it, sometimes its all in the wording.

For PEO, the payment confirmation needs to be presented. A number of people on this forum have had their PEO ILR granted without having to fill in the payment section without any issues. I think number 4 and 5 are hard to prove or provide evidence for if the person doesn't have a regular income or business of some kind.

I can see on the PEO website there are appointment dates available even in May 2015.

I'm guessing this old fee rule should still apply? if booked before the 6th April.

Provided extension was under 284 as opposed to Appendix Fm, you should be able to secure ILR once the KOLL has been completed.
