Flr(m) was applied on feb 2013. My wife's work permit dependent visa expired on jan 2013 while she was in India therefore applied for Flr(m) 13 days after the expire of her dependent visa.
She was in India for more then 18 months for health reason.

The requirement is not that you need to be in the UK in the 12 months. One of the requirements is less than 90 days outside the UK in the last 12 months before applying although discretion may be applied.
