I am also filling up Skilled worker extension form , it is also asking me the same questions. You would need to call your GPs and get the dates of visit from them. As a PBS dependant you would have paid Immigration Health Surcharge anyway!

I heard that UKBA has backlog of lots applications and hence the reason for the delay. The matter of fact, you are only allowed to submit up to 1000 Tier 2 applications in a month. Hopefully things would get better in future.

I guess, if you VISA has not expired, you could cancel the application and get your passport back from UKBA and apply in person. However, if your VISA has expired i would not recommend you to cancel, as you may have to leave the country.
