If the travel is urgent get a visa else apply for OCI
In either case Indian passport needs to be surrendered as a pre requisite.

Difficult to comment without seeing the documents - check with Italian authorities if they would accept your documents in their current form.

As per rules you can apply from any country where you are a resident (or from India). You don't need to be a citizen of the country where you are applying from. I was just taking cue from the recent trends - E-Tourist visas to French, Australian and Chinese citizens were announced during PMs visits to those countries; 10 year visa to Canadian citizens was announced in Canada.

It is my understanding that he should be given the visa for free and that all we must prove is that we are married and travelling together. I think applying as a spouse of an OCI holder would be easier than applying on his own. However include the solicitor's affidavit, school registration certificate as additional supporting info.

You can't submit on his behalf.

There shouldn't be any issue as you'll need to submit only the surrender certificate.

You'll need to provide atleast a copy of the marriage certificate. This is a material change since you applied your last passport and you'll need to provide documentation in support of that.

It's the otherway. Exit permit will be required anyway. I would skip the OCI part and apply later.
also note, exit permit is not a visa. It's issued only 72-48 hours before planned departure enabling exit from India.

1. Exit visa is required to depart from India in the below circumstances
(a) foreign national born / adopted / surrogacy in india
(b) foreign national unable to produce the original passport that was used during entry.

2. Minors are exempted.

Though Indian e-tourist visa for UK nationals is on the cards but looking at past trends it's likely to be annouced in conjuction with Indian PM's UK visit (when? it's anybody's guess!)

Not all embassies accept booking.com reservations as you don't need to make any payment for booking through this website. So check with embassies whether they have any restriction.

There isn't any concept of dependant tourist visa.
She will need to apply as a normal tourist with standard set of documents (air ticket, accommodation, medical insurance, letter from employer, proof of finance etc).
