You can apply for FLR O just before your visa is getting expired. Once you are ready to enroll into another course that is after couple of weeks you can submit and varied your application to Tier 4. In that case you will not have to go back to your country for extension.

Getting a CAS seems to be really hard these days unless ur previous immigration history is from reputable university. If you have a valid visa you can apply for Tier 2. It seems u don't have progression from your previous course so ur best bet would be applying for another category and after a year ...
