Ilr based on long residence does not permit dependents. Each person applying must meet the requirements on their own when they qualify. So it makes no difference if you apply together or not, you each still need to complete separate forms. If your children are both UK born, they do not need ilr and ...

No as the date of application is the date you submit and pay online, regardless of when your current visa expires.

Apply closer to your visa expiry date then and it should hopefully be 1200.

It is £400 per year or pro rata for part years in 6 month intervals.

That's a shame, had you stated this important fact when you asked questions in march, you would have been advised of the risk of refusal as it is mandatory for bank statements and payslips to match with salary amount.

When did the visa expire that ilr was based on??

It is irrelevant if the appointment is after visa expiry.

The only important date is the date of online application and submission. Section 3c kicks in if her visa expires as long as she applied before visa expiry.

She can apply from any country where she has legal residence and visa that is NOT as a visitor.

It means that the date that you submit and pay for your citizenship application must correspond to you being physically present in the UK exactly 5 years ago, ie not away on holiday, business etc.

Date if application is the date you submit and pay online. The date of biometric appointment with the third party agent (ie NOT the home office) is irrelevant.

IHS is not refunded when you apply for ilr.

Very little chance of getting any type of visa for a child you adopt or are "gifted" by a direct family member. The immigration rules prevent this as the bold bit I posted clearly states.
