my extention was done on 15 june 2012
however my visa expires on 21st june 2015

for 5 year period do i count back from 15th june or from 21st june ?

I have PEO appointment , and wanted to confirm if form changed, after logging on to ukba they have changed dependant form to version 06/12 but main applicant is still same

Do it at your own risk.... It actually doesn't matter what you've selected on the form. UKBA considers limited company directors as self employed.
