As other have said before, NHS is not a public benefit but is an earned benefit that comes through the National Insurance you pay.

nsm..with regards to your family, it will be the same as I have outlined before. Following your HSMP extension, you apply for FLR for yourself and family irrespective of when your family comes to the UK.

Relax...have a look at the time lines for HSMP application processing. Remember it is a waiting game something that you will have to get used to - waiting for the next extension and another one than the ILR (or probationary citizenship - sounds like a criminal sentence!!) and for citizenship.
