Finally the embassy corrected the dates and apologized for the error.

Always check your dates ! :)

How did you count 6 days as your stay? What time do you arrive into the Schengen region and on what date? What time do you depart from the Schengen region and on what date? Can you check again what under "Duration of Stay" is?
Nothing under duration of stay, it only says 5 near duration of stay

and 1 as the no of entries

how can you fill the application form with checkboxes, tried using adobe x and it seemed real pain to type one word in one box making sure its in the middle and all.
