ILR for my wife

Hi gurus,
I recently got my ILR during may. 1st week. my wife applied for tier1 PBS Extension as. She did not complete 2 years by that time in order to be eligible for ILR. She will be eligible for ILR During July 2013. Which application form should she need to use.?

Since i am self-employed, do i need to get letter from my accountant or since i am the director of the company can i issue letter for myself :D ??
