What type of contract are you on? Temp/Perm? What contract terms relate to sick pay? Oh dear. You will need that when applying for naturalisation on form AN. See pp.13 Guide and Booklet AN. Probably wise to make enquiries about getting it back - obviously someone has filed it instead of returning it to you.

Your document is issued by the authorities in India. Any changes would have to be effected by them and they determine what process - formal/simple deed poll is acceptable to grant the change.
