I think all service points are very busy at the moment.
When I booked some weeks ago there were no free appointments available and paid ones only about 2 weeks from the day I was booking. That was in the Northwest.

As above, the clock only starts from the moment the Home Office accepts the evidence that you are in a relationship akin to marriage. This is the start date of your partner's residence permit.

Yes, it was announced to all members of staff and the Home Office also gave a talk beforehand to explain what the settled scheme is about.
After that those that had interest could book a day to participate in the scheme.

Answering my own question in case someone has a similar query in the future.
Yes, the CoA allows "to accept offers of employment in the United Kingdom, or to continue in employment in the United Kingdom, whilst your application is under consideration". I received mine today.
