Yes you should include BRP.

If you are applying via NCS, they will return the BRP and Passport to you after photocopying.

MN1 is for your daughter, so all questions are about her.

Finger Print requirement is for 5 or 6 year plus kids.

You did not extend her visa (meant She is born in the UK?)

5 year Starting date

Hi,

While applying for AN1 (after 1 year of ILR and 5 year T1 (G)), is it necessary that you must be in the UK exactly 5 years before?

For Example If I am planning to apply on 20/12/2015 and I was out of UK for holidays between 15/12/2010 and 03/01/2011?

Or I have to wait till 04/01/2016?

Yes, After selecting Visa Category, Number of Applicant and their details, you can see available dates based on your search criteria.
Payment is always a last step. So don't worry.

Now a days it is quite easy. You can get appointment after 1 week approx. Why not try your self by booking, just for checking date.
