In the AN form I filled as per my current passport. And my passport has city and state both!

Yes that's what the approval letter says that I will have to attend ceremony and then request for any change on the naturalisation certificate.The only thing I am concerned about is for British Passport!

Also best to mention this on cover letter ..
We had same issue our statements date were missed in couple of previous statements. Our bank gave us a letter stating the missed dates.. and we mentioned it on a cover letter to HO too ... ILR has been granted without any issue..

You should login to your online account(internet banking), print the statements and then ask the local santander branch to certify stamp and date it. They are always happy to do it.

P60's for all 5 yrs .. this yrs yet to come so payslips X10/11 ..
letter from employer covering your out of country leaves
