Hi,

I had the same issue. Unfortunately, the form provided is in pdf ( and not ms word) format. You have to hand filled it.
I tried to converting the pdf into MS Word document using the tool available but it changed the layout of the form.

Best fill by hand then electronically.

Its Okay to have the amount in Cash ISA account. In your letter from Bank, try to get a statement that this amount was/is available to you any time.

I think even without the above statement in bank letter you should be fine but why take risk?
