When you arrange an appointment with the NCS, they will send you a passport form in the post.

A TB certificate is only required as part of a visa application. It is not required at the point of travel.

My other half has received the letter from the Home Office asking to enrol for Biometrics.

Only states to bring the letter and the one with a barcode on it. No mention of ID required.
