Is this really an issue with the BR code as I had one employee who worked 6 months full time for me on BR code.

Should this be an issue at the time of extension?

I am agreed with Meesha as your family is your dependent and you should do the maintenance of 90 days and there is no need to attach any bank statements from your dependents. I did the same thing when I applied for my family.

The best option is to find genuine employees for your business even if this is in the last 12 months and it will fulfil the extension...

I ''d recommend you to send all the supporting documents to your wife and fill the form on her behalf but select applicant filled herself and she can then print the form by logging onto website. That is the same thing i did for my wife's application.

By the looks of your question it seems to be related to Tier 1
enterprenuer. If yes then there is no probem recruiting The person having ILR and as per the rules it will fulfill the requirement and it does not matter which country he/ she is the citizen of.
