You are safe not to disclose if your conviction date is more than 5 years. Rehab period for fine is 5 years from the date of conviction.

FPN need not be mentioned.

I think they want to tally up your payslip to the bank statement to make sure you are economically active. I dont think you need all 12 months, 2-6 months payslips and bank statement should do.

It makes perfect sense to ask for a charge for signing a form by a GP. This is a legally binding document and it is taking precious time of GP at which he could seen a sick child. Also not to forget admin work that involves ie scanning document etc. This is why most GP practices do not provide this ...

You could get a letter from your accountant to support your earning. I think they are more interested in money going into your bank account on regular basis so that you can prove you have been financially independent.

It takes 5 years for a conviction to be spent. It has nothing to do with points on licence. If I were you I would declare both.
