Almost all application forms definitely ask about ones health, immigration history, crime record (if any). Wherever there is a question about health then it must be declared too which wont be the alone reason of refusal.

No. I have seen multiple posts where moderators have advised that the test centre must be on the approved list also. £150 VS rejection and loss of £3k fees.
