Bhushan, I am in the same situation( entry after 72 days of hsmp visa issue) I spoke to UKBA customer care, any thing less than three months should be fine. Re 2nd question, it shouldn't be a problem if you have enough points for extension.

Re your third question, phone nikah is not accepted for UK visas any more.
