1. Yes, since you entered UK within 90 days of EC, you can apply for ILR without second extension.

2. You can apply on or after 3rd Jan 2012, but as your current visa expires on 13 Jan 2012, you really have very small window of 10 days to apply.

I will still count both of you as lucky. I was in Croydon on 3rd and they had complete system breakdown after around 9 AM and they sent back almost 200 people back. Fees paid were refunded and so on. You need to go through SET (O) form and guidance.

For dependant childrens (not born in UK), all you need is photographs and passports and their details to be entered as dependants in SET (O) form by main applicants.

In my opinion, this should not be an issue as long as it has been clarified through the letter from employer. Completely agree with this. And I would say it is a MUST, as I remember one of the fellow forum member had his application rejected by a difference of 15p between salary slip and bank state...
