I do not think there should be any effect on your ILR, as long as there are no CCJ, or other legal proceedings against you.

1. Yes, since you entered UK within 90 days of EC, you can apply for ILR without second extension.

You need to go through SET (O) form and guidance.

For dependant childrens (not born in UK), all you need is photographs and passports and their details to be entered as dependants in SET (O) form by main applicants.

In my opinion, this should not be an issue as long as it has been clarified through the letter from employer. Completely agree with this.
