When I checked online for appointments, SET(O) now appear to have two options:

SET(O) Long residency
SET(O) excluding long residency

What do they mean, which is ILR in this?

I think this needs clarification from other seniors / moderators:

Questions:

(1) When counting 4 years, should we include the date of arrival?
(2) When counting 28 days backwards, should we include ending date?

Did you book by phone?, If so, they would have asked you the date on which 5 years complete, they would have verified 28 days, right?

01 Mar 2010 is the 29th day when counted from 01 Feb (01 Feb included). It is not an issue. Some of my visa's have the type of visa while some do not. This is normal. Just remember to submit your Home office letter that was issued alongwith the extension. Thanks VKS.

When I got my HSMP extension, the sticker on the passport had the following:

Limited Leave to Remain.
No recourse to public funds.

However, there was no mension of "Highly Skilled Migrant" in the extension sticker. Would that be a problem when I go for ILR?
