Add a cover letter and mention that you are making an application under Section 6(1) BNA and would like to be considered for that. Answer online form questions as per 6(1) residence requirements that is 5 years. You need to meet requirements as per this section if you plan to apply in your own right...

You should declare ALL CCJs even if they are 6 years old. It will NOT affect your application though if its paid/you are on a repayment plan etc. Leave the notes that you have paid it etc.
Good character requirements go back to 10 years.
