I am not familiar with the rules but all Home Office need is IHS money and as long as they get it, they shouldn't make a big issue out of it. Once you pay your IHS, send your case worker all the details including the emails you sent requesting a new link, etc. Good luck.
If you dont get any response from HO, you can pay on their website on the last date and email payment confirmation email to your case worker. Something is better than nothing. Good luck
I made IHS payement on 10-12-2018 and it was mentioned that they are updating to an advanced system or something on 15-12-2018, that might be the issue. Just wait and do it later when either it's updated or issue is resolved. Keep us posted and good luck.
It is better to throw in a cover letter. Write anything relavent you want case worker to consider which is not covered in application forms or needed explanation. Good luck
