2. The 5-year list of addresses is not related to your route to ILR or your proof of residence. I only need 3-years residence in UK to qualify for BC, but still have to input 5 years of addresses.

In my opinion it's best to take a new test. You obviously have no problem with English and paying £150 for the test gives you peace of mind. Also, bear in mind, the HO now regard the test certificate to be valid for two years only.
You should complain to the official UKVCAS email and also to HO. I agree, the whole system feels like a fiasco. Most people who applied in November and used the UKVCAS system have not received their approval yet. A lot of people who applied under the old system seem to have received their approval....

You will have received an email from UKVI confirming receipt of your application a day or two after your submitted it online. You do not get another acknowledgment from Sopra Steria or UKVI after attending your biometric appointment.
During you overseas stay did you maintain your UK residence? If yes, then I suggest it is not necessary to provide the overseas address of where you stayed. If no, then give details. It will not affect your application in my opinion.
If you apply online you use a separate downloaded form for each referee.
The paper application uses one form for two referees. I disagree with this method because the second referee will be able to see the personal data of the first referee. In this case I would still use one page for each referee.
