Could be the letter was lost of something. Could be also that they have the wrong email for you.

Go to the Council and take 2 forms of IDs with you. Your passport and driving license would do.

Don't worry about it too much. They will take your name from the passport.

But for future applications, make sure you always enter your name as on the passport to avoid surprises.

You are overthinking.

Many things in this country require a referees such as immigration applications, driving license, naturalisation... etc. People are used to sign these documents. I signed many. You are not guaranteeing anything apart that you are signing for the correct person.

It depends on factors beyond your control. If the application has not been decided when you leave the country, it will be withdrawn. Hello,

A relative applied 3 weeks ago. The visa was ready for collection within 8 to 10 days.

It was a straightforward application with many expired Schengen visas in the passport and visas for US and Canada.
