Yes a visa is required. Only those who have an EU country issued residence permit under EU Treaty Rights can travel without a visa. UK is not an EU country, and even if it is, he is not a resident in the UK so he wouldn’t have qualified anyway.

If a person who was born in Czechoslovakia and had Czechoslovakia citizenship at the time when the country split, that person is entitled to either Czech or Slovak (or in some cases, both) citizenship.
