It seems you collected surrender certificate by person. If someone provides self addresed envelope then how many do they normally take to post the surrender certificate. Please advice.

Have you got any letter from HO when you got your PR. or any old letter from HO which you received when you extended your visa previously. Check the reference no. and tell them that reference no.

just to add, if you and you wife applying together then you need show them marriage certificate. Also, you need to take with bank statement ( address proof) and you need to carry with you the ID that you provided for the LIUK test. For us, we used our Driving Licence for Life in the Uk test. Your wife needs to apply for ILR on her own by using SET(O) form. then you need to apply , i think by using FLR(M) form, need the homeoffice website. Basically you need to apply for further leave to remain for 2yrs as her dependent and then apply for ILR.
