Informally it would be called a spouse visa. More formally it would be entry clearance as a partner of a British citizen.

1 - Yes. Gap isn't a problem if you are providing 12 months of pay slips (Category B) but wouldn't work well under Category A (6 months or more with current employer). 2 - Yes. If the application is a visa for your wife, it is likely only her that would need to be in the UK at the time of application provided you were still considered 'settled' in the UK. However, if she were to leave the UK after applying her application would be considered withdrawn.

The wording of the guidance document suggests bank statements for the previous 12 months showing the income paid into the account. Any money being going out of the account the benefits are being paid into would not affect the application, they are only interested in confirming the income.
