Chances of getting ILR on that profile are very slim. In case you intend to take a chance than gather evidence of living together in UK as well as abroad. It is immaterial that what changes have been made in form.
Law is law. To be valid application should be on current version of form subject to exception mentioned at page 10 of UKBA guidance mentioned above.

Faxed copy may not be entertained and may delay the case as you are claiming points of earnings.
May be you get on 8th Aug the statement posted to you.
Otherwise get it stamped and signed by any branch of your bank.

For that give figure of latest month by including overtime.

It was not an appeal against deportation order wherein UKBA could grant FLR in pursuance of favorable order. Rather it was appeal against rejection of ILR. Hence there appears to be either an error or discretionary FLR has been given independent of appeal order . Lets wait.
