Yes I think I am going to go to the bank personally and request the same in writing and then give them a couple of calls later to make sure my request has been registered in the system.

Its difficult to not be paranoid about this :)

I did not read it completely, if you did, please share your views. I am dabbling with some pdf diff tools to find the exact differences. Wonder what would happen to poor people who applied today and did not know about this new form.

You do not need to provide any additional evidence. Just mention it clearly in your cover letter that you are assuming the payslips to be treated as evidence of UK experience and hence are claiming 5 points for the same.
