In any case I dont see any other option than to use the name present on her passport. You can explain in the additional info. IMO affidavit will be pointless. Besides she must have used her marriage certificate when she applied for dependant visa which you say has the initial for last name. If they...

Passport is the identity document, you should whatever last name is present on her passport. She is applying for naturalisation which means she must have made some UKBA applications in the past. I haven't called yet and from responses from you guys I lose hope to call and get an update :) Its really funny .... what I think is their system is not updated regularly which means if a CW has approved application he/she did...

No, there is not. You only get your original documents back shortly before or after your passport interview.
