No, They generally ask for three to four month documents covering two years. You can take that. and tell CW about the situation. Can you find any catelogue, any accountant, dentist or any sort of letter of that time.

You need to show that you are paying calss 2 national insurance as a proof of self employment activity.

You can also call HMRC and ask them to send you the welcome letter at you new address.

However, the most authentic proof is payment of Class 2 national insurance.

Totally depends at what time you are looking for the appointment. The best time is Log on to online system, at 0001 Hrs at night and then look for the appointment. I am sure you will find your desired date and time. and obviously you can only go 42 days ahead of you appointment.
