The problem is, Pakistani counsalate neither returns the passport nor they issue any letter stating passport has been cancelled. Yes. The date of application is what counts, doesn't matter how long it takes to process application.

Just send them your passport or naturalisation certificate if asked and ignore the grand parents' birth certificates. That will have no affect on outcome.

It could be anything from 6 months to a year, lately, people have been receiving decision on the same day after enrolling bio metrics as they have been processing applications and then only waiting for biometrics. Good luck.

I have read the refusal letter myself and I am pretty sure I understood every word of it. Before that, I was of opinion that all visas count towards long residence but I was wrong. I didn’t get permission to upload the refusal here otherwise I would have done so.

If you are certain that Esta was approved then you don't need a physical proof. The airlines will be able to check your Esta status with your passport number.
