The form very clearly requires you to submit this information if you are currently self-employed. Nowhere does it tell you to submit it if you were previously self-employed. Note that you don't have to send your tax forms. The only reason why someone would want to get the work history is if you don't remember it, but Home Office does not require for you to send it to them.
