Again does this mean my first ever entry (around 7 years ago) or my first entry after Tier-1 around 1 year ago?
If you have answered No to D5, then you should jump to D11 and need not answer D10?

Thanks Manoj, that sounds right. Given that the J5/J6 for NI addresses also has a country column, I guess it is a typo. May be it was meant to be County.

Tier 1 General Dependant Extension-Q J1/J2, 5 years address

Dear forum members, I have a confusion regarding questions J1/J2 of the Tier 1 General dependant extension application form (ver 04/12). The heading J says - any previous addresses lived within last 5 years, while J1 says - have you lived at any other address in the UK within the past 5 years? Confu...

GP registration should not matter at all for extension. As for your registration, ask the receptionist at the GP about who have given these guidelines and which agency can you speak to regarding the same. I was in a similar situation, called up the relevant agency, spoke to a person from the medical...

you definitely cannot apply uplift ratio for your Indian earnings since you were physically in the UK. You can only use the exchange rate for a simple INR to GBP conversion. There are umpteen cases on the forum where such use of uplift ratio resulted in rejection.
