if online shows all the details, why don't you take print and get attested. Guidelines does not mention what details should be there on payslips, it is better to show as much details and consistent across payslips to avoid confusion.

I think your rejection reasons 1 2 and 3 are more than enough to tell you what you should submit. I think you are good to go with your new documents. If you are getting consolidated payslip better get it signed and sealed. good luck.

AR is only if you are sure that the original documents supplied are enough to gain tier-1. Although the guidelines does not mention that the logo should be in colour, i think it assumes it to be so.
