No, there is no requirement for your bank account details to be printed on your payslip. As long as the net amount matches what's in your bank statement, you should be fine.

I think your chances are pretty slim. Remember, appeal is there for when you believe that the immigration officer made a mistake. In this case you didn't comply with the requirements and you refused based on the rules. The appeal officer will work with the same set of rules.

According to UKBA's site:

If you wish to rely on a joint account as evidence of available funds, you must be named on the account along with one or more other named individual.

So it should be fine, I don't see a problem according to their rules.
