If you already proved English language for ILR, you do not need to prove it again. You need to submit the same documentation you used for your ILR to prove English language. (In my case, I used the PBS printout and degree certificate and the NCS officer said that was sufficient).

It is not usually possible to process a card not present transaction without the CVV number (last three digits on the back), so you shouldn't expect to see an attempted transaction. It is the same as expecting to see an attempted transaction without providing a card number - it simply cannot be done.

There is no requirement on the age of the letter, as long as it covers all of your employment. It might even be easier getting it while your employer still has records.

Even if your employer collected tax via PAYE, not declaring this on your self-assessment probably means that your personal allowance was applied twice (once by your employer which they should do) and again when you did the self-assessment. I think you are looking at this wrong. Your council tax should be paid for the sake of not getting a criminal conviction (which will entirely destroy any chance of getting ILR soon), not for the purpose of a visa. You can cancel the appointment up to 5 days before for a full refund.

You can reschedule the appointment up to 48 hours before for no charge.

Not sure what credit you are looking for susheel - rescheduling isn't charged and you would have paid at the point you made your first appointment.
