cohabitation proof is about past and its not suggesting anything about current and future accommodation status. I think no one can predict outcome of visa here but tenancy document should have been submitted if it was valid at the time of application OR you should have tried to renew it before apply...

I would not scan signature

Applicant print form for a reason to sign it with hand..i dont know whether they will accept it if applicant or sponsor send them scan copy of signature page

It may be disproportionate for them to expect the applicant to apply for a vignette transfer and pay for their mistake. Ask them to correct the visa. Where she is applying from?

If i talk about visa centre in Pakistan then No they don't keep original form and they will keep passport and rest all supporting documents and printed form (with applicant signature on it) should be send to UKVI in Sheffield (UK) by the sponsor/applicant
