The child's name is no longer added onto mother's passport anymore and separate passport is issused for the child. The child has a claim to Indian nationality (from mother) and british nationality (from father) - however the child can't hold both passport at the same time. If you son hasn't provided...

PIO has not cancelled. The route has been closed and merged with OCI. All existing PIO card holders have been extended the benefits of OCI.
There is no need for the child to immediately apply for OCI, you can do that once the child's passport is renewed.

I don't think anyone can say it's not required as the official guidance says otherwise.
If you have insider knowledge or you have confidence on the persons that says it's not required, then you can try your luck without it.

was the visitor registered with the surgery as a private patient? if so, then the surgery would be charging automatic for consultation / treatment.
if the visitor was not registered as a private patient, then it needs to be corrected.

as per existing rules, this one year stay needs to be continuous however, the new citizenship bill will allow upto 30 days of absence, so one needs to stay 11 of 12 months in India preceeding any such application.

not all OCI holders were indian citizen's before or child of Indian citizens (such as OCI's through parent/grant parents routes, and as per new rules spouses)

hence 5(1)(g) is for people that don't qualify for 5(1)(f)

yes, it's not uncommon.
Passport is a travel document and possession of it doesn't conclusive prove the citizenship/nature of citizenship. Can't your friend apply for a replacement certificate?

...but unfortunately there is no guarantee that a post dated visa would be issued!
Inspite of the request ECO may ignore (either intentionally or by oversight) and issue visa from the approval date.
