The new good character requirement regarding periods of overstay only came into effect 1 year ago.

You should have raised the visa issue at the time, it is too late now.

You cannot appeal a refused citizenship application. You can ask for a reconsideration which costs £80 and there is no set timeframe that HO have to decide, it can take a couple of months or many months. What was the start date of your new CoS? Your new visa will likely be issued in line with that.

I have not had to do an English exam as completed my process before the requirement was introduced, so cannot comment on which is easiest. B1 level will be good as it is the level required for ILR.

From 19 November 2015, you won’t be able to use ESOL qualifications as proof for settlement and citizenship applications. This is well documented now, ESOL is no longer accepted. That aside, you still need to submit proof of LIUK and meeting the English requirement with your citizenship application...

I don't believe it will have any impact on your applications as it is only a receipt and probably not even looked at by HO (if they get a copy).

HO based their process and decisions purely on the application forms and supporting documents. NCS is only a third party handling agent.
