1) Yes. All absences during you ILR qualifying residence period should be listed.
2) Yes.
3) If it was to visit family, then that would be the reason. If you believe there's a need for cover letter you can include one, but that's a personal choice.

That's normal. Payment for each is taken separately. There's usually a gap of around a week between the two.

I'm going to go to the consulate in Manchester and see if they'll see us without having an appointment! Long shot I know... Each country consulate may be different, and do hope you do manage to see someone. Hopefully they are nothing like the Finnish embassy, who don't even let you into the premise...

I'm not sure about using the online form, when applying through the JCAP scheme, as you wouldn't be applying online. I'd pickup a form from the post office.

No, there shouldn't be. I did the same around 3 years ago. Ensure you supply the supporting documents for the B1 as your travel is business related and sponsored by your employer.
