Each applicant will have their own application form.
If both applicants travelling together then their details will require to be mentioned in the forms appropriately.

The link in my reply has explicit mentions of Tiers 2 and has the appropriate form etc. If I were you I would use that service. Either way have photocopies of the transmitted documents with yourself for records.

If the UK court gives adoption order and the adopter parent/s are British then there is no requirement for registration as adopted child becomes British and as such can apply directly to IPS for passport.
