If you get Medical allowances even if you do not actually incur expenditure on medical (and do not thus have to provide any receipts to your employer to claim the same) then it is not a reimbursement and additionally if its comes up on your every pay slips as well therefore its part of your Earnings.

First Visa 22-10-2010 granted & Visa Expire 08-10-2015. As your 1st visa granted on 22 Oct 2010 and your 5 years will be complete as on 22 Oct 2015 . You don't need another extension you can apply ILR directly 28 days before to 2 Feb 2016.
