Your 5 years continuous residence time period start from 7th Oct 2011. your current leave is expiring 21 Aug 2016. and your 5 years will be completed by 7th Oct 2016 even if you apply 28 days before to 7th Oct 2016 which is 9th September 2016 still 19 days short. sorry you need another extension to ...

You don't need another extension you can apply ILR directly 28 days before to 2 Feb 2016.

Can you please clarify this?

if you enter within 90 days grace period of time then your visa issue date will be considered for continuous residence.

If you are more concern about this issue then in addition to payslips and bank statement you need to include HMRC employment history letter which shows tax paid on amount you earned as third source.

Yes these 4 days are also coming within in 12 months claim time period so shouldn't be any problem .
salary slips from Feb-14 to Jan-15 is absolutely fine to claim if you are applying in March.
