2.2 NO as this is not the first time she applied for a visa as a spouse.
2.3 YES as this is the first time she is applying for an extension if she came in on a spousal visa valid for thirty three months. She was initially given leave to enter.

2.3 b would refer to those who came in on a fiancée visa (leave to enter) and were given 6 months to get married. As far as I know you do not have to fill in 7.3c if you are meeting the financial requirement by other means, in your case, with salary. In fact, it can cause confusion if you include other financial information.

I would answer 2.2 with NO as it is not the first time you are applying for a visa. It says 'is this the first time you have applied for a visa OR an extension.....' If you have a spousal visa, it is not the first time you have applied in that category.

I think it is six letters in both person's names, so six in joint names or twelve individually. Does she not have anything from the NHS? Or a bank account? Or Council Tax? Any official letter is acceptable and the letters must be spread over the entire time since she entered on the first visa.

I would be worried about the five months of pay slips. They can be very precise about requirements. Hopefully it will work out for you as you have no time to get the sixth payslip. I do not understand though the statement that your wife's visa is valid until 2017. Maybe I am missing something.

Appendix 2 is not submitted online but is sent with your printed application form and supporting documents. So download and print it, fill in by hand and submit. Don't forget to keep a copy for yourself!
