I believe for T1, you need to meet the point requirements by using a max of 12 payslips from the past 15 month period. So not necessary to have the most recent payslip included. Few questions,
1. Did case worker explicitly ask for documents above?
2. Did you have to submit latest payslip and bank statement, I presume it was for December 2012?
3. Did cw ask anything about absences?
4. Did cs require letter from employee?

Do not get too stressed out, wpilr_nov12 gave you a hint above, you must have had another reason(s) for this journey.

Better be, but if you mean "mini-statements from automatic teller machines (ATMs)", they are not permitted as per rules.

Local branch should help, try to find decent person from staff and ask for print outs, they do this in exceptional cases, bear in mind they should be on bank headed paper, stamped and signed. Generally banks should do this with no hassle for last three months.
