I don't think there will be any problem. Just keep ur old passport and marriage document with u when ever u travel.
At the time of visa extension in the application form just fill the section ' if u r known by any other name' and the new visa will be with new name.

The Biometric is only one type the data capturing process for goverment record to prevent and control illegal immigrant.
Each and every applicant has to go through this process and then only the application is processed.

Yes u need to take TB certificate for ur 4 month old daughter but only from certain register clinics. Click the following link.. read it and on the right corner of the page there is a list of clinics for TB screening from india. Dont forget ur are switching from PSW to Tier 2 General.
The start day as the day after the last day of visa expiered is for the person who is already having tier 2 visa for same company and appling for extend to leave on tier 2.

Generally the process time is 4 to 6 week after biometrics. your biometrics was done on 7th of march so it should be nearly on final stage but as u said ur cos is showing assigned. You should apply as soon as possible, and before your current permission to stay expires.
Make sure for ur code of practice and salary beforer cos assign . check the new soc 2010 for proper code and salary.
